[Early action without dilating the pupil: for early detection and early consultation]
◯ You may see opacities in the jelly-like “vitreous” that fills the eye
◯ Dots, rings, clouds, cobwebs, “flashes of light,” and so on
“Floaters”
◯ The vitreous changes with age and can sometimes tear a hole in the retina: rhegmatogenous retinal detachment
✔ If you notice a sudden change, it is important to see an eye doctor as soon as possible.
✔ High-definition observation of the fundus is now possible without pupil dilation (without widening the pupil):Ultra-widefield fundus imaging

Physiological floaters (normal)

Retinal tear / retinal detachment

Causes of retinal tears and retinal detachment[36-second video]
・The inside of the eye is filled with a jelly-like substance, the “vitreous.”
・The vitreous, which sits loosely against the retina, shrinks and liquefies with age.
・A hole can form in the retina where the vitreous is strongly attached to it (retinal tear).
・Once a retinal tear forms, fluid enters beneath the retina. This is a “retinal detachment.”
Floaters (first half) and retinal detachment (second half)[35-second video]
・Light passes through the transparent “vitreous” and reaches the retina.
・Small clumps of cells or strands can form in the vitreous and cast shadows on the retina.
・Dots, rings, clouds, cobwebs, and so on
・With age, the vitreous can shrink and create a small hole (tear) in the retina.
・A new floater may appear as a result.
・When the vitreous pulls the retina out of position, you may keep seeing flashes of light (this can lead to headaches).
・Dangerous signs of retinal detachment:
− Flashes of light
− A sudden new floater
− A shadow at the edge of your vision
− A curtain-like shape moving across your vision
・If you notice any of these, you need to see an eye doctor as soon as possible.
Vitrectomy for retinal detachment[23-second video]
・The vitreous is removed and the inside of the eye is replaced with fluid.
・Silicone oil may be used as needed, and reoperation may be required.
How we see with the retina[46-second video]
・Light passes through the cornea, pupil, lens, and vitreous and reaches the retina.
・In the center of the retina, the “macula,” and the surrounding retina, light is converted into signals, which travel through the optic nerve to convey information to the brain.
