(You can take your time filling it in.)
We understand that visiting an eye clinic for the first time can be very worrying.
Searching for medical information online can sometimes make that worry worse.
We listen to the history leading up to your current symptoms and explain a care plan tailored to you.
So that things can move in the best possible direction:
Please feel free to share any questions, concerns, or requests — no matter how minor they may seem.
- What is happening in your eyes right now
- The cause of the condition / treatment options
- Whether surgery is needed, etc.
Understanding the eye can sometimes be difficult.
The image a word brings to mind may lead to a different understanding from what was intended.
Unresolved questions can cause anxiety to grow.
We give detailed explanations using images until you feel you understand.
Please ask everything you want to know.
We explain things so that your worries are kept to a minimum.
“Is there anything else you would like to ask?”
: we ask this at the end of every consultation.
Please bring your health insurance card and any medical subsidy certificates, and, if you are seeing other doctors, your medication record book (okusuri techo).
* Please arrive with plenty of time to spare so that we can take the time to listen to you carefully.
* If an explanation takes a long time, we may ask you to wait briefly before resuming the discussion.
* If you are in a hurry, please let the reception staff know.
(Please understand that tests appropriate to your symptoms may take some time.)

We will explain anything about your eyes that is unclear to you.
Please keep asking until it makes sense.
If you need a further explanation on another day, please let us know at any time.
Explanations over the phone may not be sufficient, so we ask that you visit the clinic in person.

Cover one eye and look at the black dot in the center.

Upon arrival, please provide the following information to the reception staff.
