A Lifestyle with “Long Periods of Near Viewing”

We see many people, from children to adults: people who study a lot, people who do long PC work in the office, performers, people who make crafts, people who draw pictures and illustrations.

What they have in common:“Long hours looking at near objects.”

We provide care focused on the focusing muscle (the ciliary muscle).

Diagram of accommodation (how the eye focuses): when looking far away the ciliary muscle relaxes and the lens becomes thinner; when looking near the ciliary muscle contracts and the lens becomes rounder

A lifestyle with “long periods of near viewing”

[Eye tests and examination]

○ Visual acuity test: measures the refractive state (the power of the eye)

○ Accommoref (accommodation analyzer)
For about 50 seconds per eye, it quantifies the tremor of the ciliary muscle (high frequency component, HFC) as you view a fireworks target approaching from infinity to 1 m to 50 cm to 33 cm, and shows it in a graph.

○ Examination with a slit-lamp microscope, and screening for other eye diseases

* In particular, we check for the presence of glaucoma, retinal disease, and cataract.
* Fluorescein staining: observing the state of dry eye (the three-layer structure of the tears).

We show the results on a vertical monitor placed in front of you, and mainly do the following.

[Eye drop prescriptions]

Cycloplegic: Midrin®-M (tropicamide) once a day at bedtime

Acts on the parasympathetic nervous system and relaxes the ciliary muscle.

* Midrin®-M eye drops: there is no evidence that they slow myopia progression in children.

Cycloplegic: atropine (diluted) once a day

Atropine completely paralyzes accommodation by blocking the parasympathetic nervous system.

It is normally used for detailed testing of strabismus (esotropia) and amblyopia, and in the acute phase of iridocyclitis and uveitis.

If used as is, the pupil dilates for 1–2 weeks, which is not suitable for daily life (= mydriasis).

Diluting it 100-fold almost eliminates pupil dilation.

We expect a stronger effect than Midrin®-M.

In growing children, it also has the effect of slowing the lengthening of the axial length (myopia progression control)。

 * Myopine®is now also available. Please contact us for details.

Accommodation improver: Miopin® (neostigmine methylsulfate) three times a day

Acts on the parasympathetic nervous system and helps the contraction function of the ciliary muscle.

Dry eyeeye drops

Unlike the impression given by the name “dry eye,”
there are many symptoms other than “dry eyes.”

An unstable tear film is thought to be a cause of eye strain.

Dry eye

Reducing inflammation is what works first.

Others include artificial tears and mucin / water secretion–promoting eye drops.

[Glasses and contact lens prescriptions]

○ Glasses or contact lenses made long ago may be overcorrecting.
Distance is clear, but the ciliary muscle becomes very tense when looking at near objects.

○ “I’ve had good eyesight since I was young”
This is often mild hyperopia, and the ciliary muscle becomes very tense when looking at near objects.

Even among near tasks, the distance people look at seems to differ from person to person.

• Long time looking at a smartphone / social media
• Studying for entrance exams
• PC work (computers)
• Playing music while reading sheet music
• Drawing delicate, detailed pictures
• Taking photos through a viewfinder, etc.

Considering the condition of your eyes and your living situation,
we think together about glasses and contact lenses that suit each person’s life.

I now spend a lot of time thinking about the causes of myopia (refraction) and eye strain.

Besides the nuisance of glasses and contact lenses,
if myopia progresses, eye disease can develop in adulthood (pathologic myopia)。

If eye strain continues, productivity falls, leading to a social loss as a result.

“I want to reduce myopia in children.” “I want to reduce the disruption to everyone’s work.”

A big difference from when I worked at hospitals is that in Takatori I can follow people over a long period.
I also want to take a positive approach to care that fits the local community.
